Vine Youth is a youth group of Macquarie Anglican. KYCK is a conference run by the Katoomba Christian Convention.

Please register your child for KYCK19 by completing this registration form and then paying the $125 registration fee online at the end.

By accepting these conditions and submitting this form:

I permit my child to fully participate in activities associated with KYCK19 from 12-14 April 2019.

I permit photographs and video of my child to be taken while participating in KYCK19 to be used by Macquarie Anglican to promote Vine Youth and its camps.

I permit, in the case of a medical emergency, the doctor chosen by a Vine Youth Leader and/or Macquarie Anglican Staff to secure proper treatment for and/or order hospitalisation, injection, anaesthetic, or surgery for my child as named. I understand that every effort will be made to contact me prior to instituting such procedures.

I acknowledge and agree that children attending KYCK19 must be signed in and out by a parent/caregiver. I agree to advise Ryan Carter of the names of any other authorised adults who can sign my child in and out.

I confirm that the information given in this form is correct. I agree to advise Ryan Carter of any changes to this information.

I understand that my child will be attending KYCK19 under the supervision of Vine Youth Leaders and possibly Macquarie Anglican Staff.
