We respect your privacy. When you provide us with personal information by participating in church events/activities, visiting a website, or otherwise, you agree to this personal information about you and your children (under 18 years old) being handled as set out in our privacy policy.

The personal information in this form will be made available to the Zest For Life Holiday Club Leaders, who will treat this personal information confidentially. The personal information in this form may also be shared with medical and emergency services if considered necessary.

Authorisations and Expectations

I permit my child to participate in activities associated with Zest For Life Holiday Kids Club (9 July - 11 July 2025) at Macquarie Anglican, unless I advise the Zest leaders otherwise.

I authorise the Zest for Life leaders, in the event of an emergency, to obtain at my expense any medical, ambulance, rescue or other services that are considered necessary for my child.

I acknowledge that being part of a community involves mutual care and consideration, and therefore agree, that unacceptable behaviour may result in my child being sent home and being temporarily or permanently prohibited from attending the activities of Zest for Life 2025.

I also acknowledge and agree that any photographs or video of my child taken while participating in Zest For Life may be used by the church internally (i.e. during a church service, on church premises) to promote further childrens activities. Photos would not be published online without my consent.

I understand children attending Zest For Life must be signed in and out by the Family/Emergency Contacts listed above. I will notify the Zest leaders (at the sign in desk) of the names of any other adults who will be picking up my child.
